Admissions to the inpatient unit are prioritised on individual need.
You can call the Hospice on 020 8343 8841 for details or you can ask your GP, hospital consultant, district nurse or similar medical professional to speak to us on your behalf.
A referral form will need to be completed - anyone can do this although it's best if it is done by a medical professional as we require details of your medical records.
Health professionals wishing to make a referral should use the Hospice referral form and fax it to 020 8343 7672.
