FAMILY SUPPORT WORKER (37.5 hrs)
Salary: NJC scale 25-28, £28,215 – £29,766 p.a.

Following investment in End of Life care from the CCGs in Barnet, Enfield and Haringey as part of the North Central London Sustainability and Transformation Partnership we are extending our multi-disciplinary community teams to enable us to provide psychosocial support to more people in the last year of life, and those important to them.
The new investment will mean we will offer support to people earlier on in their illness helping them to develop advanced care plans to ensure their wishes and preferences are met and they are fully supported throughout their journey.
We are seeking an experienced social care professional to provide additional psychosocial support in our Barnet Community team (approx. 22.5 hrs) and our 18 bed In-patient Unit (approx. 15 hrs) It is full-time role and the person appointed will work across both teams, based at our North Finchley site. As the post involves community work car owner and driver is essential.
The role of Family Support Worker is newly developed
You will work closely with the social work team to assist and support them in their work and facilitate timely, creative, person centred support, that enables people to make informed choice and manage their health, wellbeing and lives independently, for as long as possible.
