Salary: £20,367 – £22, 705 p.a. (actual) pro rata of NJC 34- 38

Following investment in End of Life care from the CCGs in Barnet, Enfield and Haringey as part of the North Central London Sustainability and Transformation Partnership we are extending our multi-disciplinary community teams to enable us to provide psychosocial support to more people in the last year of life, and those important to them.
The new investment will mean we will offer support to people earlier on in their illness helping them to develop advanced care plans to ensure their wishes and preferences are met and they are fully supported throughout their journey.
We are seeking two experienced Social Workers to provide additional support in our Barnet and Haringey Community teams (22.5 hrs).
There are two part-time roles and the person appointed will work closely with the other members of the multi-disciplinary teams, including social workers and family support workers.
As the post involves community work a car owner and driver is essential.
You will work with people to facilitate timely, creative, person centred support, that enables people to make informed choice and manage their health, wellbeing and lives independently, for as long as possible.
