SOCIAL WORKER (22.5 hrs)

Salary: NJC 34-38, £19,059 – £21,018 p.a. actual salary

Following investment in End of Life care from the CCGs in Barnet, Enfield and Haringey as part of the North Central London Sustainability and Transformation Partnership we are extending our multi-disciplinary community teams to enable us to provide psychosocial support to more people in the last year of life, and those important to them.

The new investment will mean we will offer support to people earlier on in their illness helping them to develop advanced care plans to ensure their wishes and preferences are met and they are fully supported throughout their journey.

We are seeking an experienced, qualified social worker to provide additional social work support to our multi-disciplinary community team in Barnet, based at our Finchley site, Woodside Avenue, N12. As this is a community role a car driver is essential.

You will:

respond to complex need

use critical analytical skills to inform assessments and interventions

work in partnership with service users and multi-disciplinary colleagues to promote enablement and support people to achieve personal outcomes/objectives

provide safe, timely, effective and innovative psychosocial support to patients, families and carers
