Following investment in End of Life care from the CCGs in Barnet, Enfield and Haringey as part of the North Central London Sustainability and Transformation Partnership we are extending our multi-disciplinary community teams to enable us to provide psychosocial support to more people in the last year of life, and those important to them.

The new investment will mean we will offer support to people earlier on in their illness helping them to develop advanced care plans to ensure their wishes and preferences are met and they are fully supported throughout their journey.

Social Workers

We are seeking experienced, qualified social workers to join our teams:

Barnet Community team (based at Finchley site)                      22.5 hrs

Enfield Community team (based at Winchmore Hill site)            22.5 hrs

Haringey Community team (based at St Ann’s Hospital site)      37.5 hrs

In-Patient Unit (18 beds based at Finchley site)                        37.5 hrs

 

You will use critical analytical skills to inform assessments and interventions, respond to complex psychosocial needs, work with risk and uncertainty and promote choice, control and self-determination.

 

Family Support Workers

This is a newly developed role and we are seeking to recruit one worker to each site:

Barnet Community team (based at Finchley site)                      22.5 hrs

Enfield Community team (based at Winchmore Hill site)            22.5 hrs

Haringey Community team (based at St Anne’s Hospital site     22.5 hrs

You will work closely with the social work team to assist and support them in their work and facilitate timely, creative, person centred support, that enables people to make informed choice and manage their health, wellbeing and lives independently, for as long as possible.
