Palliative Care Clinical Nurse Specialist – Health and Wellbeing Team
Band 7 – £44,346 – £50,548 p.a. (inclusive of HCA)
A fantastic opportunity has arisen for a dynamic, enthusiastic, self-motivated and experienced Palliative Care Clinical Nurse Specialist to join our Health & Wellbeing (H&W) Team at our Health and Wellbeing Centre, based at Winchmore Hill. You will be joining a multi-disciplinary team who are passionate and have extensive knowledge of supporting those living with and affected by a life limiting illness. The Health and Wellbeing Centre provides a range of outpatient’s services to support with symptom management, rehabilitation and self-management, psychological and emotional needs and the wellbeing of patients and their families.
As part of this role, you will act as a resource of clinical knowledge and expertise for individuals, their family and friends, health care professionals and other agencies.
Your responsibilities will include undertaking a holistic needs assessment, planning and delivery of a consistently high standard of specialist palliative and supportive care in accordance with the Hospice policy, procedures and standards.
You will also support the delivery of the H&W service across both Winchmore Hill and Finchley sites including providing a cross-site clinic service alternating with the Team Lead.
The post holder will hold a degree and a Masters in Palliative Care/Oncology or be willing to work towards a Masters. With a minimum of five years post-registration experience, two years of which should be in palliative care/oncology setting (hospital or community) and a year of which should be at senior-nurse level, i.e. Band 6 or Band 7.
We are looking for a candidate who is passionate about palliative care, understands the needs of those receiving palliative care or learning to live well as a consequence of their treatments or long term survivorship issues has a working knowledge of evidence-based practice, an understanding of current initiatives and developments within palliative and end-of-life care, excellent communication skills and the ability to work as part of a multi-disciplinary team.
